PRESCRIPTIONS & REFILLS

Please call us if you have questions about your prescription or referral. All refills and referral questions should be asked during normal office hours to enable us to review your medical record. You may also call your pharmacy with any of your medical concerns.

VISITS

We generally see children from birth until 18 years of age or graduation from high school, whichever occurs last. We would be happy to see your child for any medical concerns or questions. We also strongly encourage your children to have regular exams and immunizations as recommended by the American Academy of Pediatrics. Please call in advance to make these appointments, especially in the summer.

TELEPHONE CALLS

We accept most of the major health insurance plans as well as visits on a self-pay basis. Please bring your insurance card to your visit.

Any co-pays are required at the time of service. Also, we request that any outstanding balance on your account is settled before the next visit. We accept cash, personal checks, debit cards, MasterCard, Visa, and AMEX. There will be a charge for all returned checks. If you cannot make prompt payment, please discuss the matter with our billing staff so an arrangement can be made.

Insurance plans vary, and you are responsible for any costs that are not covered by them. Questions or complaints about your coverage, co-pays, or deductibles should be directed to your insurance carrier.


PRIVACY STATEMENT & MEDICAL RECORDS

Currently, we have privacy policies that conform to those required by federal law (HIPAA). Your medical record will be held strictly confidential.

If you are new to this clinic, it would be very helpful if any previous and pertinent medical information for your child can be transferred to us. Also, if you need to change clinics, we are happy to release a copy of the medical record to your new doctor for a smooth transition of care. Lastly, there is a nominal charge if you request copies of physical forms or immunization records.
